Who is eligible for Teesside Hospice care?
Patients with active, progressive and advanced disease including non-malignant are eligible for hospice services.
Below are links to the relevant referral forms. Please note that only health professionals can submit a referral to our services.
We can offer an outreach visit to introduce hospice services, which includes both In-Patient Unit and Wellbeing Centre. To learn more about our services and how we can best support you, visit the links below:
